The goal of the program is to assist mentally ill individuals in accessing services needed to enhance their quality of life while allowing them to be part of the community at the most productive level possible for that individual. This program utilizes a wrap-around model of service delivery designed specifically for children and adults to assist with linking individuals and families to formal and informal supports and services in the community. Case management can help in the area of living arrangements, physical health needs, mental health needs, economic needs, vocational/educational needs, social functioning, family functioning, self-care needs, self-direction, and substance abuse.

Also provides trauma care for individuals having mental health difficulties related to current and/or past abuse experiences, including victims, survivors, and offenders. Offers an Injection Clinic.

Offers three community residences, with one focusing on individuals with both a mental illness and a chemical addiction (dual diagnosis), with 24 hour supervision that are licensed by the New York State Mental Health.

Provides a centralized intake process for adults and children who have significant behavioral health needs that would benefit from additional supports and services in the community. This includes people with significant behavioral and medical or social risk factors which can be addressed through overall care management. There are three types of SPOAs - Children's, Adult Case/Care Management and Adult Housing. The SPOA process provides for the identification of people most in need of services, and manages service access and utilization.
