Description

Provides non-emergency, in-home mental health assessments to older individuals. Short-term supportive counseling can be provided to address current life issues including depression, transportation needs, grief, confusion, isolation and loneliness, anxiety, and other issues. Case managers are assigned to help individuals make linkages to other programs and services available in the community.
