Single Point of Access (SPOA) | Steuben County Community Mental Health Center

Provides a centralized intake process for adults and children who have significant behavioral health needs that would benefit from additional supports and services in the community. This includes people with significant behavioral and medical or social risk factors which can be addressed through overall care management. Children's Services (CFTSS & CORE) | Catholic Charities Steuben/Livingston - Livingston Site

Provides Children and Family Treatment and Support Services (CFTSS) to children and youth under the age of 21 who are covered by Medicaid and are enrolled in a Medicaid Managed Care plan. Referrals may be made by parents, teachers, doctors, or anyone connected to the youth.

Recovery Coordination | Pathways, Inc. (Admin Site)

Assists adults Medicaid Managed Care to gain peers and partners in the community. The Care Manager provides ongoing assessment and referral for Adult Behavioral Health Home and Community-Based Services (HCBS). These services are for adults aged 21 and up who are Health and Recovery Plan (HARP) eligible with behavioral health needs. The adult may decide to be referred for coordination services to assist with HCBS and other needs, or the Care Manager can continue to monitor HCBS involvement.
