Must complete referral form. Go to website for form, select Adult or Children SPOA along left side. Scroll to bottom of page to access link for (Adult) A-SPOA referral form or (Child) C-SPOA Application. Provides a centralized intake process for adults and children who have significant behavioral health including medical and/or social risk factors. There are three types of SPOAs - Children's, Adult Case/Care Management and Adult Housing. The SPOA process provides for the identification of people most in need of services, and manages service access and utilization.
