The supportive apartment program supplements the needs of people who are living on their own, but who may need periodic financial and staff supports to retain their independent living. Also offers a respite program to families who are responsible for the daily care of a loved one with a developmental disability and need relief.

Provides care management to individuals with a serious mental illness, HIV/AIDS, or two or more chronic health conditions and significant behavioral, medical, and/or social risk factors, and who are in need of assistance linking to services to stabilize and improve their overall health functioning.
