Provides help for high risk youth facing running and human trafficking issues.

Provides Children and Family Treatment and Support Services (CFTSS) to children and youth under the age of 21 who are covered by Medicaid and are enrolled in a Medicaid Managed Care plan. Referrals may be made by parents, teachers, doctors, or anyone connected to the youth.

Provides supportive case management services to adults and children that are severely and persistently mentally ill. The goal of the program is to assist mentally ill individuals in accessing services needed to enhance their quality of life while allowing them to be part of the community at the most productive level possible for that individual. This program utilizes a wrap-around model of service delivery designed specifically for children and adults to assist with linking individuals and families to formal and informal supports and services in the community. Case management can help in the area of living arrangements, physical health needs, mental health needs, economic needs, vocational/educational needs, social functioning, family functioning, self-care needs, self-direction, and substance abuse.

Cases are assigned to workers who meet with families on a regular basis and work closely with schools regarding children who are experiencing difficulty with behavior or who have emotional difficulties. Works with individuals referred by the Steuben County Department of Social Services (DSS). Services provided may be either preventative or transitional and are meant to provide short-term services in order for the child to remain at home and be successful. The program also helps youth who have been in placement with transition back with their families and communities, as well as to their home schools. For those transition cases, services can begin while the youth is still in placement, in order to help prepare parents for their child’s return to the home.

Focuses on the goals of the family to address complex emotional and behavioral needs of the youth, and partners with the family and facilitates the intensive process with the collaborative team, including natural supports and community providers, while honoring family voice. The team builds the family-specific strengths to get needs met, learn new skills, and develop skills and supports to manage crisis.
