Provides integrated, supportive housing for people with disabilities who are facing homelessness. Apartments are available for Medicaid recipients 55 and older who require assistance with activities of daily living; individuals who are 18 and older with eligibility for services through the NYS Office for People with Developmental Disabilities (OPWDD); and military veterans with disabilities. Staff is located on site who advocate for residents; connect them to services available in the community; provide independent living skills training, and facilitate social and recreational opportunities.

This program is a collaboration between Pro Action's Family Development Program, the Woodhull Community and Family Resource Center, Addison Jennie Mose Family Resource Center, Corning Nonnie Hood Parent Resource Center, and Bath Area Family Resource Center.
