Medicaid Service Coordination (MSC) | Mozaic

Provides assistance to individuals with developmental disabilities in gaining access to the necessary support and aid needed to live productive lives. Uses person-centered planning focusing on the needs and desires of the individual receiving services while striving for the highest quality of life. Develops an Individualized Service Plan (ISP) which outlines what goals the individual hopes to accomplish, the strategies to achieve these goals, and what support they will receive along the way. Plans are evaluated and updated as the individual progresses and to ensure all support and services are meeting the individual's needs. Also assists individuals with services such as future planning, residential planning, recreation, residential habilitation, day habilitation, reimbursement, adaptive equipment, and environmental modifications.

Program services include outreach and engagement, comprehensive care management with assessment and care planning, care coordination and health promotion, comprehensive transitional care, client and family support, referral to community and social support services, and use of health information technology to link to services.
