Programs are responsive to new and changing interests of people receiving supports and their families. The menu of available supports focus on social skills, daily living skills, wellness, technology, music, and the arts. Participants are fully included in their communities through recreation and experiential learning.

Provides non-medical in-home services including help with bathing, eating, shopping, cooking, cleaning, and laundry based on need. The service also includes a care manager whose responsibility is to determine the individual's needs.

Treats adults and adolescents in a supportive and therapeutic environment. Offers individual and group therapy groups, crisis services, medication evaluation, treatment and management.

Assists youth and caregivers get the services they need to improve the youth’s health. As part of the Health Home Care Management model, through CHHUNY, LLC, a care manager coordinates, advocates, and educates to support children and youth who are enrolled in Medicaid and have qualifying health conditions.

Provides Care Management services in supporting a person’s recovery including coordination of an individual’s medical, mental health, substance abuse, and social service needs.

Uses person-centered planning focusing on the needs and desires of the individual receiving services while striving for the highest quality of life. Develops an Individualized Service Plan (ISP) which outlines what goals the individual hopes to accomplish, the strategies to achieve these goals, and what support they will receive along the way. Plans are evaluated and updated as the individual progresses and to ensure all support and services are meeting the individual's needs. Also assists individuals with services such as future planning, residential planning, recreation, residential habilitation, day habilitation, reimbursement, adaptive equipment, and environmental modifications.
