Provides non-medical in-home services including help with bathing, eating, shopping, cooking, cleaning, and laundry based on need. The service also includes a care manager whose responsibility is to determine the individual's needs.

Assists adults Medicaid Managed Care to gain peers and partners in the community. The Care Manager provides ongoing assessment and referral for Adult Behavioral Health Home and Community-Based Services (HCBS). These services are for adults aged 21 and up who are Health and Recovery Plan (HARP) eligible with behavioral health needs. The adult may decide to be referred for coordination services to assist with HCBS and other needs, or the Care Manager can continue to monitor HCBS involvement.

Offers an outpatient treatment facility for adults and adolescents that want to overcome substance use disorders including chemical use and dependency, as well as internet addiction and gambling addiction. Provides evaluations/assessments, group therapy, individual counseling, case management, consultations, toxicology screenings, community referrals, follow-up care/transitional services, family counseling, and relapse prevention groups. Medication Assisted Therapy (MAT) is also available for alcohol, opiate (synthetic or heroin), and/or nicotine dependence.

Provides patients with care following an acute care hospitalization, when the patient needs more time to recover from a surgery, illness or injury. Includes a private room, skilled nursing care, physical therapy, occupational therapy, speech therapy, care management, respiratory therapy, nutritional counseling, activities and compassionate care.

Provides Service Coordination, Community Integration Counseling, Positive Behavioral Interventions and Supports, and Independent Living Skills Training as an alternative to institutionalization.

Provides coordinated transitional services and training to help students move from school to post-school life in order to live as independently and self-directing as possible. Services include transitional assistance with CSE meetings and IEP transition planning and advocacy with school personnel as needed. Also allows participants to explore and observe a variety of jobs and work settings, including job training and coaching.

Provides information and referral, independent living skills instruction, peer support services, advocacy, and community integration services to individuals with disabilities in order to empower them to live within the community and maintain independence.

Community Supports and Day Services are designed to provide enriching opportunities for people with developmental disabilities to enhance their social skills and their independence. Programs are responsive to new and changing interests of people receiving supports and their families. The menu of available supports focus on social skills, daily living skills, wellness, technology, music, and the arts. Participants are fully included in their communities through recreation and experiential learning.

Facilitates a community center offering a single-point of entry to all ACCORD services and partner agency services including referrals to other programs and services and follow-up care for individuals in need. Assistance completing home heating applications, referrals and budgeting assistance with utilities. Food pantry on-site.

Provides case management to individuals with one or more chronic health conditions and significant behavioral, medical, and/or social risk factors, and who are in need of assistance linking to services to stabilize and improve their overall health functioning.

Program services include outreach and engagement, comprehensive care management with assessment and care planning, care coordination and health promotion, comprehensive transitional care, client and family support, referral to community and social support services, and use of health information technology to link to services.

Treats adults and adolescents in a supportive and therapeutic environment. Offers individual and group therapy groups, crisis services, medication evaluation, treatment and management.

Aids individuals with intellectual and developmental disabilities in making choices and finding individualized services. Provides help in navigating through available services including assistance with eligibility determination, service coordination, and advocacy services. Also helps families with children who have developmental disabilities transition from children's services through adult services.

Assists families who are caring for a relative with a developmental disability at home by aiding the caregiver and thereby enhancing family stability. The goals of the program are to maintain family unity, prevent premature or inappropriate out-of-home placement, enhance parenting skills, maximize the potential of the family member with a developmental disability, and provide quality of life.
