Alternative to Incarceration Program | Chemung Project for Bail

Provides help with bail expediting, court advocacy, and court ordered restrictions including home monitoring and supervision. Offers information and referral regarding the legal system and services that may be available to help individuals stay out of the justice system. Assists in overcoming barriers that have blocked self-sufficiency. Case managers work with individuals one-on-one and in group settings with substance abuse, literacy, transportation, and/or mental health.

Recovery Coordination | Pathways, Inc. (Admin Site)

Assists adults Medicaid Managed Care to gain peers and partners in the community. The Care Manager provides ongoing assessment and referral for Adult Behavioral Health Home and Community-Based Services (HCBS). These services are for adults aged 21 and up who are Health and Recovery Plan (HARP) eligible with behavioral health needs. The adult may decide to be referred for coordination services to assist with HCBS and other needs, or the Care Manager can continue to monitor HCBS involvement.

Treatment Program | CASA-Trinity of Hornell - Outpatient

Offers an outpatient treatment facility for adults and adolescents that want to overcome substance use disorders including chemical use and dependency, as well as internet addiction and gambling addiction. Provides evaluations/assessments, group therapy, individual counseling, case management, consultations, toxicology screenings, community referrals, follow-up care/transitional services, family counseling, and relapse prevention groups.
