Community Supports and Day Services are designed to provide enriching opportunities for people with developmental disabilities to enhance their social skills and their independence. The menu of available supports focus on social skills, daily living skills, wellness, technology, music, and the arts. Participants are fully included in their communities through recreation and experiential learning.

Offers a variety of day services and activities for individuals with developmental disabilities including out of home respite for children to provide the caregiver a short break, social and recreational programs and in home habilitation.

Offers assistance in ensuring healthy pregnancies and child birth including Medicaid and WIC enrollment, prenatal health education, advocacy and assistance in eliminating social barriers to healthy pregnancies (transportation, housing and other issues), childbirth classes, and referrals to other services within the community.

Responds and investigates reports of adult and child abuse or neglect. Investigates and intervenes on behalf of adults who are unable to act on their own behalf, manage their own affairs, or who are in immediate danger. Ongoing supervision for those at risk of abuse is also available.

Works with participating school districts through Greater Southern Tier BOCES to provide classroom speakers, field trips, business site visits, internships, mentoring, mock interviews, architectural awareness, job shadowing, career days, and various school to work initiatives.

Family Court Act - tries to keep situations out of court. Probation Officers process and investigate youths who have been in trouble by being truant from school, runaway or incorrigible who require supervision. Works with client and family to resolve the situation by mentoring, guidance or will refer to out side counselling. Parent or legal guardian can refer youths to the PINS program.

Provides child care referrals, child care subsidies, provider training and career counseling, administers child and adult care food program, registrar for family day care and school age programs, enrollment of legally exempt child care providers, and technical assistance to employers regarding child care services.

Assists with pursuing OPWDD eligibility status determination, self-advocacy, and person center planning, provides funding assistance for disability related services and supplies and recreation opportunities within the community for disability enrolled individuals and family members.

The Office for the Aging offers many services for the senior residents of Steuben County including, but not limited to, information and referral, nutrition through congregate meals, advocacy, support groups, transportation services, caregiver services, case management, and the Bone Builders and Senior Meal Programs. Bone Builders and the Senior Dinner Clubs are overseen by Pro Action of Steuben-Yates.

Provides early childhood education and family support services to children and families. The program goal is to offer a wide range of social and educational experiences to help prepare children and families for a successful transition into kindergarten. Also offers home visitors to families of program participants and to families who do not have a Head Start within their school district. Visits are designed to strengthen family protective factors and encourage family resilience using supportive services, education on child development, offering community referrals, and supporting families with their goals.

Provides electronic equipment which connects frail elderly individuals or people who have disabilities with participating hospitals, paramedics or other sources of emergency assistance. A unit is connected via the telephone line and a pendant is worn by the individual. If the person needs assistance, they press the button on the pendant and the unit dials 911 who will dispatch emergency responders. If the smoke detector is set off, the unit dials 911 and they dispatch the fire department. The system will work if the person is outside within 500 feet of the base unit.

Provides educational workshops and seminars, caregiver discussion group, counseling, respite care, resource library, information and assistance on a wide variety of services and topics. Also provides information and assistance for grandparents and other relatives age 55+ who are caring for children.

Provides parenting skills classes to help individuals understand how to better deal with issues that arise with raising children.

Provides supportive case management services to adults and children that are severely and persistently mentally ill. The goal of the program is to assist mentally ill individuals in accessing services needed to enhance their quality of life while allowing them to be part of the community at the most productive level possible for that individual. This program utilizes a wrap-around model of service delivery designed specifically for children and adults to assist with linking individuals and families to formal and informal supports and services in the community. Case management can help in the area of living arrangements, physical health needs, mental health needs, economic needs, vocational/educational needs, social functioning, family functioning, self-care needs, self-direction, and substance abuse.

Provides integrated, supportive housing for people with disabilities who are facing homelessness. Apartments are available for Medicaid recipients 55 and older who require assistance with activities of daily living; individuals who are 18 and older with eligibility for services through the NYS Office for People with Developmental Disabilities (OPWDD); and military veterans with disabilities. Staff is located on site who advocate for residents; connect them to services available in the community; provide independent living skills training, and facilitate social and recreational opportunities.

Provides friendly visiting services, help with shopping and errands, and occasional household chores. Household chores does not include minor home repairs. Offers non-emergency medical transportation to seniors for in-town medical appointments in Bath.

Provides case management to individuals with one or more chronic health conditions and significant behavioral, medical, and/or social risk factors, and who are in need of assistance linking to services to stabilize and improve their overall health functioning.

Provides caregivers a temporary break from care giving, while the person with Alzheimer’s disease continues to receive care in a safe and supportive environment. Scholarships are available.

Focuses on the goals of the family to address complex emotional and behavioral needs of the youth, and partners with the family and facilitates the intensive process with the collaborative team, including natural supports and community providers, while honoring family voice. The team builds the family-specific strengths to get needs met, learn new skills, and develop skills and supports to manage crisis.
