Works collaboratively with state agency, local partners, and families to identify solutions. Serves children, youth, and young adults from birth to age 22 with physical, emotional, developmental, and/or intellectual disabilities who have multiple care needs and are experiencing difficulty accessing services. Works with state and local agencies and parents to identify and resolve inter-agency issues and jurisdictional disputes that impede access to services. The primary objectives are to ensure that individual children receive the most appropriate community-based or residential services; minimize delays in arranging services; resolve service disputes; and facilitate the development of specific services and programs designed to meet the needs of children and youth with non-traditional or complex needs.
