Community Based Services | Clarity Wellness Community - Wellsville Clinic

Provides a variety of family-support services. Provides Home-Based Intervention services for individuals aged 5-21. Provides supportive case management services to adults and children that are severely and persistently mentally ill. The goal of the program is to assist mentally ill individuals in accessing services needed to enhance their quality of life while allowing them to be part of the community at the most productive level possible for that individual. This program utilizes a wrap-around model of service delivery designed specifically for children and adults to assist with linking individuals and families to formal and informal supports and services in the community. Case management can help in the area of living arrangements, physical health needs, mental health needs, economic needs, vocational/educational needs, social functioning, family functioning, self-care needs, self-direction, and substance abuse.
